When visiting our office, please bring the information marked in the chart below about the Custodial Parent (CP), Noncustodial Parent (NCP), and Child(ren). Services are available without providing all of the requested information; however, the more accurate information we receive, the easier it will be to collect child support.

* Please tell our office when you move. Child support cases follow the custodial party to any new County or State of residence.

** Social Security Number Disclosure: Our office is required to collect Social Security Numbers. Social Security Numbers are kept on file to establish, modify, and enforce child support orders. Enrolling a child in health insurance may require the release of the child's Social Security Number and mailing address to the other parent's employer or the other parent. Collecting Social Security Numbers is mandatory under federal law. You can still get services even if you do not have a Social Security Number.
