Paternity Opportunity Program (POP)

POP image Through the Paternity Opportunity Program an unmarried couple can legally establish paternity by completing a paternity declaration form. Both signatures must be witnessed by authorized agents, preferably at the time of birth, in the hospital, or otherwise, at any Department of Child Support Services, vital statistics or welfare office, or Family Law Facilitator.

As of January 1, 1997, it is necessary for unmarried parents to complete and sign a paternity declaration or obtain a judgment of paternity by a court in order to have the father's name listed on the birth certificate.
